2008 Joint Conference on Health Registration Information

We are asking all JCH attendees to register on-line this year. Anyone who does not have access to the Internet may call Kathy Kondakjian at
(425) 337-5717 to register. Registrations received after 9/22/08 will be subject to the late fee. If your registration is being processed through your accounting office this may cause a long delay; you may want to register on-line and indicate on your registration that your accounting office is processing payment.  
All concurrent session presenters will need to have a registration form sent in on-line.

You will need to use Netscape or Internet Explorer to submit online forms to this website.

Payment: If you wish to pay by check or purchase order, please mail to: WSPHA, 4617 144th Place SE, Snohomish, WA 98296-6916.  Purchase orders can be faxed to (425) 357-9694 but registrations must still be entered on-line. Paying by credit card is an option you can select once you’ve completed the on-line registration form. You may want to print out the on-line form for your records. An electronic copy is immediately sent to Kathy Kondakjian and she will know how it is being paid by the selection you make on your form. Credit card payment confirmations will also go directly to her for any credit card payments made. Netscape or Internet Explorer must be used to process credit card registrations and payments together.

Pre-registration will close this year on Wednesday, October 1st.  If you are unable to get your registration submitted on-line before the 10/1 date you will need to register at the conference. Walk-in registration are welcome, we ask that you bring payment with you or have a billing address for invoicing

Cancellation Policy: All cancellations must be done in writing or e-mailed to Kathy Kondakjian no later than 9/22/08, no exceptions. A $25 processing fee is charged for all cancellations. "No Shows" will result in forfeiture of entire fee. Substitutions are permitted with prior notification

Meals (breakfast and lunch) are part of the registration and cannot be subtracted from the registration fee.

Special Membership Rate for Joint Conference Registrants!  As a registrant of this year’s Joint Conference you qualify for a special membership rate of $75 which includes dual membership in both the Washington State Public Health Association and Washington State Environmental Health Association.  Individually these members are $50 each but for attendees of the 2008 JCH you can obtain membership in both organizations for one full year at the rate of only $75.  Click here for membership information and membership application.

CEHA Pre-Conference Session - Sunday, October 6th: The Community Environmental Health Assessment Workshop will be held again in 2008 at the JCH.  More details for this session will be added to the website at a later date.
